• Flight Application:
  • applicable on US Airways Express, US Airways Shuttle, America West Airlines, America West  Express operated flights.
  • NOT VALID ON US codeshare flights, HP codeshare flights 
  • Travel via HP codeshare flights operated by Big Sky Airlines (GQ) is permitted on connecting service
    only (HP* to HP, HP to HP*). No local Big Sky code share flights allowed.
• Reservation & Ticketing: 
  • Reservation for other than the first flight on must be made at least 24 hours prior to departure.  
  • Airpass for sale when pax holds an international ticket. For sale outside USA
    
  • Only one set of Airpass can be issued in conjunction with each trip beginning and ending outside the US/Canada/ Bahamas/ Bermuda/ Puerto Rico/ Caribbean/ US Virgin Islands.
    
  • Tickets must be marked "NON- ENDORSABLE" 
    
  • Tickets may be open provided the routing is specified on the ticket.                     
  • One coupon is required for each flight boarded. (Noted: Direct flights require one coupon)                  
  • A coupon must be issued for each flight independently if its is a transfer connection or a stopover
• Child / Infant: One child or infant fare per paying adult travelling together
• Stopovers:
  -  Only two stopovers may be made at any one point; however, a point may be transited additional times for connection purposes.
  -  Pax traveling from one stopover point to another via an on-line connection, have a maximum time of 12 hours. Any connection over 12 hours will constitute a stopover.
  -  The following airports are considered the same stopover points:
  Boston                  : BED - BOS
  Washington           : BWI - DCA - IAD
  New York              : EWR - HPN - ISP - JFK - LGA - SWF
  Miami                   : MIA - FLL
  Norfolk                  : ORK - PH
  Los Angeles          : BUR - LGB - LAX - ONT - SNA

  San Francisco       : OAK - SJC - SFO
  -  A maximum of 2 one-way Transcontinental trips without en-route stopovers are permitted
  -  A Transcontinental trip is a trip between a poin in Coloumn A and a point in Column B:

• Combinations: Combination of the First class VUSA is not permitted with Economy Class VUSA Fares.
  Open jaws trips are permitted.
• Booking Class: Economy class: T class on US & HP operated flights
  First class: A on all US & HP  operated flights
  Exception: if "A" class is not available, or not offered, pax must be booked in "Y" class,                              however no refund will be due the purchaser.
• Cancellation / Refund: - Before departure, full refund will be made less USD115 service charge.
  - Of any portion of the VUSA ticket has been used, no refund will be made
• Rerouting / Flights changes :
  • Provided VUSA ticket are reissued at least 1 days before departure of the new flight . Changes in origin / stopover / destination points are subject to USD 115 reissuance charge
• Add On Fares :
                          - Travel between USA and Alaska / Hawaii / Caribbean / Mexico / Central America / US Virgin                           Island / San Juan available for additional surcharge per flight segment
  - Adult and child the same rate
  - Use the same booking class as for the Airpass
  - Surcharge applies for each flight segment for travel to / from the following markets:

Note: Children's fare apply for children 2 - 11 accompanied by an adult passenger paying a fare governed by  this rule. Only one child per fare paying adult permitted. Unaccompanied children 5 - 11 years of age must pay the published adult fare shown in this rule.

• Airport taxes, Airline Insurance taxes, and Fuel Surcharges are not included in the airfares displayed because they may be subject to frequent revisions by airlines and also they may be subject to currency fluctuations.
  Taxes and Fuel Surcharges can only be finalized and confirmed at the point of ticket issuance

• Customer are requested to kindly checked directly with the country embassies concerned to ensure fully Entry Visa(s) Compliance.
• Customer have to undertake entire responsibility to ensure that they have meet fully with a Entry Visa(s) requirements of the countries or destinations that they are traveling to.

• Prices shown are in US dollars [US$] and subject to a service fee of RM100.00 per person
